Ottawa Valley Health Connect is supported by the Ottawa Valley Ontario Health Team, serving nearly 80,000 people in the Renfrew County and South Algonquin region.
Our Mission
Ottawa Valley Health Connect is here to ensure you and your loved ones stay safe, healthy and supported at home and in the community by connecting you with local programs and services that meet your needs.
We believe that access to the right support at the right time makes all the difference. Our goal is to make navigating community health services simple, accessible and stress-free for older adults and their caregivers across the Ottawa Valley.
What We Do
We connect older adults and their families to over 190 local programs and services including:
- Home support and personal care
- Meal delivery and nutrition programs
- Transportation services
- Social and recreational activities
- Fall prevention programs
- Caregiver support
- Seniors Active Living Centres

Our coverage area
The OVOHT is one of 57 Ontario Health Teams approved by the Ministry of Health across the province.
Under OHTs, hospitals, physicians, homecare, community health, and social service providers work as one coordinated and integrated team, no matter where they provide care.

Many services are free of charge. However, some services such as personal support, transportation, meal delivery and adult day programs have an associated fee. Any fees will be noted in the service listing.

Yes. Family members, caregivers, friends and healthcare providers can request services on behalf of someone else.
